Job Title: CASE MANAGER FIELD

Job Description

We are seeking a Targeted Case Manager with experience to provide supportive services to our consumers. We welcome all qualified professionals who want to be part of our team of dedicated and compassionate employees:

Responsibilities:

  • The Targeted Case Manager will have performed assessment, service plan, provides care coordination, support to medically needed consumers who are in need of additional support to help coordinate mental health and physical health services.
  • Works alongside the Case Manager in identifying the needs of consumers who have co-occurring mental illness and significant medical issues.
  • Provides Care Coordination with the consumers and his/her mental health and physical health providers.
  • Assists the consumer in completing tasks needed for medical concerns, i.e. completing required lab work, tests needed for physical health illnesses, etc…
  • When needed, attend appointments with consumer and his/her doctor to help coordinate care
  • Acts as a bridge for consumers accessing medical care for chronic physical health conditions
  • Provides assistance and support in assisting consumers in establishing care for physical and mental health conditions
  • Acts as primary provider for consumers who are stepping down from higher levels of case management
  • Provides assessment and referrals for consumers who require services to remain independent in the community.
  • Reach out to referrals to assess level of need for case management services.
  • Provide services to Children and adult population.

Requirements:

  • Must have a Bachelor degree from an accredited University, with preferred experience in the human service.
  • Experience as a Targeted Case Manager at lest 1 year.
  • Excellent interpersonal and communication skills, including both oral and written.
  • Ability to establish effective working relationships with both Center staff and various community agencies.
  • Ability to facilitate planning meetings with individuals from multiple systems.
  • Proficient in use of technology, including Microsoft Office.
  • Provide and maintain dependable transportation and auto insurance.
  • Ability to build and maintain close working relationships with clients and community partners.
  • Relevant experience in mental health emergency services.
  • Complies with all relevant Center policies and procedures.
  • Performs other related duties as deemed appropriate by immediate supervisor or designate.
  • Ability to recognize and be sensitive to cultural and ethnic differences.
  • Must be able to safely perform essential job functions, with or without reasonable accommodations.
  • Clearance AHCA background level 2.
  • CEU'S: Bipolar disorder, schizophrenia, CFars, Fars, Substance abuse, Depression, Domestic violence, HiPPA Compliance.

Salary: $37,500.00 to $41,500.00 /year
